Sometimes you or someone you love has improved to the point of no
longer needing acute hospital care but returning home is not yet
an option.
The Transitional Care Unit bridges the gap between the hospital's
acute care setting and the patient's home allowing for a smooth transition.
The TCU provides the Southern Ocean County Hospital care you trust
in a positive environment.
The Transitional Care Unit is a 20-bed skilled nursing unit, located
on the second floor of the James J. Mancini Pavilion. It is designed
to meet the needs of people requiring short term, skilled nursing
care or less intensive rehabilitation care.

The goal of the Transitional Care
Unit is to assist you in fully recovering from your acute episode
by providing the nursing and rehabilitative care necessary to help
residents regain a certain level of independence and return home
with confidence.

Our Staff...
Our dedicated staff delivers the same
quality care you've come to expect at Southern Ocean County Hospital.
A team of nurses, physical, occupational, and respiratory therapists,
dietitians, aides, social workers and activities staff work with
the resident and family members to develop and manage each resident's
care plan. This system of careful consultation and monitoring assures
each resident of receiving the most appropriate, highest quality
of care available.
About Our Services...
The philosophy of care at
the TCU is progressive in nature. Services are all geared toward
one objective - restoring your physical and emotional strength so
you can return to your independent lifestyle. In addition to skilled
nursing care, the TCU offers physical, occupational and speech therapy
as well as therapeutic recreation. Residents are encouraged to move
about the facilities and participate in a range of activities.
Your comfort is also a primary concern. You are encouraged to wear
your favorite street clothes throughout your stay. In general, we
have found that dressing for the day's events promotes a sense of
wellbeing and restores confidence. Likewise, dining with others in
our tastefully appointed dining room will help lift your spirits
and renew your strength.
What Conditions Are Appropriate For The
Transitional Care Unit?
The Transitional Care Unit is ideal
for those people requiring short-term skilled care who are medically
stable, and those who are possible candidates for intensive acute
rehabilitation services.
Patients with cerebrovascular disease, cardiopulmonary disorders,
orthopedic disabilities, neuromuscular disease, oncology disorders,
or general debilitation are some examples of those who would be considered
for an evaluation.
The TCU Difference

SOCH's Transitional Care Unit from the Acute Care setting and other
Skilled Nursing Facilities:
- Convenience- Unlike a separate skilled
nursing facility, services such as pharmacy, IV therapy, radiology,
and laboratory are all on site, in the hospital.
- Cost-Effectiveness- One very important difference in these cost-conscious
times is that because TCU care is less intensive, it is less expensive
than an acute care hospital bed.
- Encourages Independence- Every action of the health care team helps
residents move towards one goal: acquiring the skills and physical
ability to return to their pre-hospital setting.
