Specially trained, compassionate staff provide care, guidance, resources and skilled assistance to people who are recovering from illness or injury; or those who are struggling with a chronic disease. Each patient benefits from an initial assessment from a certified home health nurse, who will develop an individualized plan of care. The nurse will work closely with each patient's physician, provide ongoing monitoring of care and act as an advocate to assure the family has access to any additional services they may need. Certified Home Health Aides, under the supervision of the registered nurse, will provide personal care, oversee the patient's nutritional needs and monitor the patient's medication compliance. The Home Health Aide will also assist with grocery shopping, meal planning/preparation, and light household chores.
