Falls Prevention
Initiative
Patients in the hospital who may have
difficulty walking due to illness, injury or because they are
on certain medications are at higher risk for falling if they
attempt to get out of bed without assistance. A fall can
lead to further injury and additional health complications, therefore,
SOCH has implemented a comprehensive program to reduce the incidence
of patient falls.
Patients who are at risk for falling
have a lighthouse magnet placed outside their room, as well as
a picture of a lighthouse above their bed. In addition,
a bright orange sticker is placed on their chart and a bright
orange ID band is placed on their wrist. These indicators
help staff and volunteers throughout the hospital identify patients
at risk for falling, allowing them to take special precautions
when taking patients to another part of the hospital for procedures
and tests.
These strategies have worked. Since implementing the program,
SOCH has lowered the incidence of falls to below the national average.
